11/29/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C engaged Ashiyah in a discussion to determine if there are any new behaviors since the last session. Ashiyah verbalized to QMHP-C she had an altercation with her girlfriend so that was a little upsetting to her. 2. Behavior: Ashiyah participated in the session and did not need any redirection from QMHP-C. Ashiyah was educated on the importance of her communicating her feelings to eliminate symptoms of depression and other behaviors that causes her to become upset and engage in the behavior she engages in. 1. Intervention: QMHP-C began session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C held 1:1 with Ashiyah about forming alliances with female peers she once had issues with in order to tease or taunt another female peer who was allegedly âtalking about her.â QMHP-C processed with her about how she feels when peers are saying things about her and helped her understand how she was now doing the same thing. 2. Intervention: QMHP-C allowed Ashiyah to take personal space due to negative responses to redirection regarding poor peer relations and limited ability to understand her role in conflict. QMHP-C her to consider the feelings of others and to put herself in their shoes regarding gossip and trying to get others not to like someone just because she has issues with them. 1. Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Ashiyah displayed oppositional behaviors toward QMHP-C when questioned about her role in conflict. She denied saying anything to the peer and said she isnât responsible for what other people say to her. Ashiyah asked QMHP-C why she acted like she didnât believe her and why did she keep asking her a lot of questions 2. Response: Ashiyah had difficulty accepting her role in the situation; she acknowledged and understood what QMHP-C was saying but said she didnât make them dislike the peer as that was their own choice. She said people should be able to make their own decisions and not follow behind her. QMHP-C Interpretation of clientâs progress towards todayâs objectives: Ashiyah continues to struggle with being able to express her feelings and emotions in an assertive manner. QMHP-C Plan of Action: VACCD staff will continue to teach Ashiyah alternatives to assist her with implementing healthy outlets through the use of role playing and modeling 1-2 times (Weekly).
